To nominate someone for the Second Annual Tennis Industry HALL OF FAME, please complete this form before July 1st, 2009.
All entries will be anonymously recorded for review by the TIA Nominating Committee.

Criteria:
• Impact / Time period: 1960s to present
• Individuals do not necessarily have to be TIA members or representatives from member companies
• Categories include: Inventors, Founders, Innovators and Contributors who are well-recognized within their field for their accomplishments
Inventors: those who changed the game through product, design, etc.
Founders: those who started noteworthy groups / organizations that made and continue to make a difference
Innovators: those who marketed tennis with a noticeable difference in how it was perceived / growth attained
Contributors: those who impacted and touched lives to make tennis a better sport
